Caine in John Wick is a blind retired assassin and an old friend of John Wick, played by Donnie Yen in John Wick: Chapter 4 (2023), directed by Chad Stahelski. He appears only in that film, where the Marquis forces him out of retirement to hunt John by threatening his daughter.

Who Is Caine in John Wick?
Caine is a blind retired assassin and an old friend of John Wick, and he is credited in the films under that single name. Donnie Yen plays him in John Wick 4, where he is billed second, directly behind Keanu Reeves. John Wick: Chapter 4 (2023) was directed by Chad Stahelski, who also directed every earlier main film in the series.
So, who is the blind guy in John Wick? It is Caine. He is introduced as a man who has already left the trade, which matters because the story has no use for him until the Marquis, played by Bill Skarsgård, finds a way to pull him back in. Caine is not in John Wick (2014), John Wick 2, John Wick 3 or Ballerina John Wick, so his whole screen history sits inside one 170 minute film.
Why is Caine blind, and why do searches ask whether he gave his eyes? The plot outline of Chapter 4 calls him a blind retired assassin and states no cause, so any explanation beyond that would be a guess. Cassian, a rival assassin in Chapter 2 played by Common, is a different character, and the two should not be mixed up.

Caine's Story Arc in John Wick: Chapter 4Spoilers
Caine's arc runs from reluctant hunter to reluctant ally, and it is driven entirely by his daughter. After John kills the Elder, the Marquis takes charge of the hunt with the full backing of the High Table. He strips Winston of his role, destroys the New York Continental and has Charon killed. He then forces Caine out of retirement by threatening Caine's daughter, which leaves Caine with no real choice about whether to track John.
Caine's trail leads to the Osaka Continental, run by John's friend Shimazu. Waves of High Table enforcers hit the hotel, and Shimazu dies protecting John. In the middle of that fight Caine spares Shimazu's daughter Akira, which is the first sign that he is not the Marquis's willing instrument.
Later, in Berlin, John needs a crime family to sponsor him for a formal duel with the Marquis. Katia, who leads the Ruska Roma, makes him kill Killa Harkan first, and John does it with help from Caine and a bounty hunter called the Tracker. By then Caine is fighting beside the man he was sent to kill. The Marquis then names a reluctant Caine as his stand-in for the duel at sunrise at Sacré-Cœur in Paris.
Caine's Relationships: John, the Marquis and His Daughter
Caine's most important relationships are with John Wick, the Marquis and his daughter, and each pulls him in a different direction. John is an old friend, so the hunt is personal for Caine in a way it never is for the Marquis. Chapter 4 builds its Paris finale on that bond, because Caine is named the Marquis's stand-in and ends up opposite John in the duel.
The Marquis is the opposite kind of figure: a young, ruthless aristocrat who has the High Table behind him. He forces Caine to hunt John by threatening Caine's daughter. That threat is the whole engine of Caine's involvement, and it explains why a retired killer takes the job at all.
Then there are the people around John. Winston acts as John's second in the duel, with his own life tied to the result. Charon is killed early in the Marquis's campaign, and the Bowery King stands with Winston at the end. For the lead himself, see the John Wick character page. Caine also shares the film with Akira, Shimazu's daughter, whose fate becomes tied to his in the closing scene. For everyone else, browse the John Wick characters index.
What Happens to Caine at the End of John Wick 4?Spoilers
Caine survives John Wick: Chapter 4, and the film leaves him free but with an open threat. In the duel at Sacré-Cœur he wounds John, but Winston points out that John has not yet fired his third shot. John then shoots and kills the Marquis. The Harbinger honors the result by freeing John, Caine and Caine's daughter from the High Table, and by restoring Winston to his position.
Caine's last appearance comes in the post credit scene. He reunites with his daughter in Paris. Akira, seeking revenge for her father's death, approaches him with a knife. The film does not show what happens next, so Caine ends alive, released from the High Table and facing a personal reckoning that stays unresolved on screen.
What about a follow up? TheWrap and Screen Daily reported in April 2025, around Lionsgate's CinemaCon presentation, that a spin-off built around Donnie Yen's character was set to shoot that summer, and that coverage gives no release date. The main saga's next entry, John Wick 5, is in development at Lionsgate with Keanu Reeves and Chad Stahelski, with no release date announced.
Caine's Best Moments, Ranked by Opinion
In my opinion, Caine's best moment is not a fight at all but the decision to spare Akira at the Osaka Continental. It could be argued that this single choice tells the audience everything about him before the film asks us to forgive him, and it sets up the closing knife scene far better than any speech could.
Second, perhaps, is the Berlin sequence in which Caine and the Tracker help John with Killa Harkan. It works as a team up set piece, and it is the point where the film stops treating Caine as an obstacle and starts treating him as a partner with his own stakes.
Third is the duel at Sacré-Cœur. Caine is forced to stand opposite his friend, and the moment he wounds John is staged as a loss for both men, not a win for either. That being said, the Rue Foyatier steps belong to John, and Caine's role is to make the final shot cost something.
If you are new to the saga, start with the John Wick movies in order and treat Chapter 4 as the payoff to that run. Fans who want a ranking of the films themselves can see where Chapter 4 lands on the John Wick movies ranked page, and the John Wick timeline shows where Caine's story sits.
